Assisted Living Waiver (ALW) Program Manager

Our interdisciplinary team of caring healthcare professionals are here to assist with the physical, psychosocial, spiritual and bereavement needs of our patients.
When there is a chronic condition, life-ending illness, or simply additional care for your loved one is needed, we are here to assist.
Our aim is to help navigate families during what can be an overwhelming journey.
We create a support system that allows families to focus on their loved ones while we coordinate and manage their care.
We want to ensure that our patients and families feel loved, safe and cared for.
We are patient ADVOCATES before we are employees.
Responsibilities:
Determining Medi-cal eligibility of clients Submitting ALW Assessments to DHCS Scheduling RN/Social worker visits with clients Maintaining accurate client records Communicating with DHCS regularly Communicating updates with families, clients, Assisted Living Facilities, Skilled Nursing facilies etc.
Assisting with billing as needed
Experience:
- Previous experience with Assisted Living Waiver Program is required This job description is not exhaustive and additional responsibilities may be assigned as needed.
